Foot Surgery: Options and Approaches

Advanced foot surgery encompasses a range of surgical interventions designed to address complex foot and ankle conditions that have not responded to conservative treatment. These procedures are performed to alleviate pain, restore function, reduce deformity, and prevent further complications.

The conditions we address surgically are wide-ranging. A contracted Achilles tendon, called equinus contracture, is a root cause of many mechanically related problems and can contribute to heel pain, stress fractures, ball of foot pain, mechanical arthritis, acquired flatfoot, and forefoot wounds in patients with diabetes. Ankle arthritis, most commonly caused by prior fractures or sprains, can progress to end-stage disease. Ankle instability, cartilage lesions, ruptured tendons, bunions, hammertoes, and severe deformities may also require surgical correction.

Common symptoms include persistent joint-line pain, swelling with activity, locking or catching, recurrent giving way of the ankle, visible deformity, and pain that limits daily walking. Repetitive activity in demanding sports like soccer or basketball can also produce a gradual build-up of tissue without an obvious single injury.

If you or someone you know is experiencing ongoing pain, instability, or deformity in the foot or ankle that has not responded to rest, bracing, or therapy, seek out the help of a podiatrist for a proper diagnosis and treatment plan.

Surgical Techniques and Recovery

Our surgical approach is matched to the specific problem. Ankle arthroscopy uses small incisions and a telescopic camera to treat joint adhesions, small fractures, bone spurring, and soft tissue build-up, permitting early range of motion and reduced recovery time. Subtalar arthroscopy addresses persistent problems deep in the back of the ankle, including bone spurs, cartilage lesions, loose bodies, and synovitis.

For Achilles problems, gastrocnemius recession lengthens the calf through a small incision, while direct Achilles tendon lengthening uses three small incisions when greater length is needed. Ruptured tendons are repaired by suturing the torn ends and following a structured protocol that progresses from splinting to a CAM boot with heel lift, then weight-bearing as tolerated, and finally physical therapy over roughly 8 to 12 weeks.

For arthritis and instability, ankle fusion removes remaining cartilage and stabilizes the joint so the bone grows together, reducing disabling pain while preserving the ability to walk. Ankle distraction arthroplasty is an alternative for younger patients, using a circular external fixator for three months to stimulate cartilage regeneration. Brostrom repair and ligament reconstruction with tendon graft restore stability after chronic sprains.

Minimally invasive foot surgery can correct bunions, hammertoes, stiff big toe, painful calluses, and heel pain through very small incisions, with less soft tissue injury, less pain, and less scarring. Most procedures are outpatient, and we will guide your recovery from immobilization through return to full activity.
